Your First Appointment

There's a lot of anxiety around your first appointment--its a major step for you, and in some cases there may be little or no experience with therapy in the past. It's important that every client is informed and prepared to start their treatment with us. 

WHAT TO EXPECT: THE INTAKE ASSESSMENT
The first appointment is called an intake assessment. It is an information gathering appointment, where your provider gets to know you, your past, your current symptoms and concerns, and gains important knowledge that will aide in your treatment. This appointment is the basis for all future appointments, as it helps to establish a diagnosis and direction for treatment, based on your input. 

HOW LONG WILL THE APPOINTMENT LAST?: Your first appointment is typically about 1 hour and 15 minutes. 


WHEN TO ARRIVE: 15 minutes early to complete any necessary paperwork. If you arrive late, you will still need to complete this essential paperwork, which may shorten the time you have with your provider. 

WHAT TO BRING: Please bring your insurance card, medication list, any custody related documentation (if treatment is for a child 13 years of age and younger), and a form of payment for any out of pocket cost your insurance dictates.
For telepsychiatry medication management services, you will also need to have your medical and mental health records faxed to 570-748-5717 (our psychiatrist may not prescribe medications on the first visit if these records are not received).  

WHAT TO DO PRIOR TO YOUR APPOINTMENT: Please complete the below paperwork prior to your appointment. You can submit this paperwork electronically back to us by emailing keystone_counsel@comcast.net, or you can bring it with you on the day of your appointment. If you do not arrive with this paperwork, you will need to complete it on the day of the appointment.

ABOUT THE PAPERWORK: The file below will help your provider get to know you quicker. If you are a parent or guardian, please complete the paperwork with information specific to your child that is presenting to treatment. For adolescents, they may wish to complete portions of this paperwork on their own, in private.

WHAT WE DO PRIOR TO YOUR APPOINTMENT: We will reach out to your insurance carrier, ensuring you are set up to start treatment and inquiring about your benefits. We will contact you before the first appointment to inform you of your benefits, which will include cost information.
